(a) He/she shall agree to the creation, alteration or modification of the equipment, provided said procedure suits a specific need.
(b) He/she shall participate in and agree to the creation, alteration or modification of the equipment.
(c) He/she shall release the professional from any liability for damages that may arise from the use of the equipment thus created, altered or modified.
(d) He/she shall make good use of the equipment thus created, modified or altered.
(e) He/she shall see to the maintenance of the equipment pursuant to the specifications established by the professional who altered or modified the same.
