Section 388-828-4040 - What scales are contained in the supports intensity scale (SIS) assessment?

The supports intensity scale assessment contains the following:

(1) The support needs scale;
(2) The supplemental protection and advocacy scale;
(3) Exceptional medical support needs scale; and
(4) Exceptional behavioral support needs scale.
