Section 388-828-4020 - What is the purpose of the supports intensity scale (SIS) assessment?

The purpose of the supports intensity scale assessment in the DDA assessment is to determine all of the following:

(1) Your ICF/IID level of care score for DDA HCBS waiver eligibility;
(2) The health and welfare needs that must be addressed in your individual support plan if you are enrolled in a DDA HCBS waiver;
(3) Your DDA behavioral and medical acuity levels regardless of your age; and
(4) Your support need acuity levels specific to the:
(a) DDA activities of daily living acuity scale;
(b) DDA interpersonal support acuity scale; and
(c) DDA mobility acuity scale.
