Section Res 2402.04 - Form EDP-1, Certification Form(a) A municipality, county, or organization requesting certification shall supply on Form EDP-1 the following information:(1) Name, address and telephone number of the municipality, county, or organization; and(2) Name, title, address and telephone number of the contact person.(b) All information shall be typewritten.(c) The municipality or county requesting certification shall include a copy of its incorporating documents describing the form of government the municipality or county operates under, if applicable.(d) The organization requesting certification shall include a copy of its bylaws or charter, if applicable.(e) The contact person representing the municipality, county, or organization shall: (1) Sign and date the form; and(2) Certify by his or her signature that all final reports due on previously funded projects have been completed and submitted.(f) The municipality, county, or organization shall mail or hand-deliver the completed certification form to: Department of Resources and Economic Development
