Failure to provide any of the information for certification shall result in denial of eligibility unless good cause for the lack of such information exists. Good cause shall include, but not be limited to, loss of information due to fire, theft, or other cause beyond the control of the applicant. Claims of good cause shall be made in writing to the screening committee and the commissioner.
