Section Pos 701.03 - Institutional Application(a) Each institutional applicant to participate under RSA 188-D:18-f through RSA 188-D:18-h in the workforce incentive program shall complete and execute a commission participation agreement, form PEC 700.(b) The institutional applicant shall supply the following information on the participation agreement: (1) The name of the institution;(2) The amount of funds accepted and to be matched; and(3) The name and title of institution's authorized official.(c) The institution's authorized official shall sign and date the participation agreement.(d) The institution shall also submit: (1) A current certificate of good standing issued by the New Hampshire secretary of state; and(2) A certificate of authority stating who is authorized to sign on behalf of the institution.N.H.
