An application or complaint initiated pursuant to NRS 704.300 for the elimination, alteration, addition or change of a highway crossing or crossings over any railroad at grade, or above or below grade, including its approaches and surface, must be made in writing to the Commission. The application or complaint must contain the information required by NAC 703.450, except that the crossing number of the affected crossing must be stated in addition to the information required by subsection 2 of NAC 703.450.
