Section 403-2-005 - LEVEL OF CARE DETERMINATION005.01INITIAL. Prior to receiving services under this Title, an individual must be determined by the Department to meet the Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) Level of Care.005.02REDETERMINATION. To remain eligible for services, an individual's status must be reviewed and the Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID) Level of Care determined: (A) Within 12 months of the previous level of care determination;(B) No earlier than 60 days prior to the implementation of a renewed Individual Support Plan; and(C) At any time there is a significant change in a condition affecting an individual's level of care.403 Neb.
