Section 11-172-04 - Filing of a provider application

Each applicant seeking initial certification or provider seeking re-certification under this chapter shall file an application with the department. The application shall be filed on the form prescribed and furnished by the department. The department shall not accept for filing any application that is not submitted on the correct form.
