The application form prescribed by the department may vary according to the type of service being proposed in the application. The department may revise or amend the application forms from time to time. An application form may require the applicant to provide any information that is reasonably necessary for an informed review of the application, and the applicant shall provide the information in the form and manner that the department may prescribe. The department may require the applicant to provide supporting document, information submitted in the application.
