Section 6 CCR 1007-1-4.45 - Records of Planned Special Exposures4.45.1 For each use of the provisions of 4.11 for planned special exposures, the licensee or registrant shall maintain records that describe: 4.45.1.1 The exceptional circumstances requiring the use of a planned special exposure; and4.45.1.2 The name of the management official who authorized the planned special exposure and a copy of the signed authorization; and4.45.1.3 What actions were necessary; and4.45.1.4 Why the actions were necessary; and4.45.1.5 What precautions were taken to assure that doses were maintained ALARA; and4.45.1.6 What individual and collective doses were expected to result; and4.45.1.7 The doses actually received in the planned special exposure.4.45.2 The licensee or registrant shall retain the records until the Department terminates each pertinent license or registration requiring these records.4.45.3 Upon termination of the license or registration, the licensee or registrant shall permanently store records on Department Form R-16 or equivalent, or shall make provision with the Department for transfer to the Department.Colorado Register, Vol 37, No. 14.
