Section 6 CCR 1007-1-4.46 - Records of Individual Monitoring Results4.46.1 Recordkeeping Requirement. Each licensee or registrant shall maintain records of doses received by all individuals for whom monitoring was required pursuant to 4.18, and records of doses received during planned special exposures, accidents, and emergency conditions. Assessments of dose equivalent and records made using units in effect before January 1, 1994 need not be changed. These records shall include, when applicable: 4.46.1.1 The deep dose equivalent to the whole body, lens dose equivalent, shallow dose equivalent to the skin, and shallow dose equivalent to the extremities;4.46.1.2 The estimated intake of radionuclides (see 4.7);4.46.1.3 The committed effective dose equivalent assigned to the intake of radionuclides;4.46.1.4 The specific information used to assess and calculate the committed effective dose equivalent pursuant to 4.9.1 and 4.9.3, and when required by 4.18;4.46.1.5 The total effective dose equivalent when required by 4.7; and4.46.1.6 The total of the deep dose equivalent and the committed dose to the organ receiving the highest total dose.4.46.2 Recordkeeping Frequency. The licensee or registrant shall make entries of the records specified in 4.46.1 at intervals not to exceed 1 year.
4.46.3 Recordkeeping Format. The licensee or registrant shall maintain the records specified in 4.46.1 on Department Form R-17, Occupational Exposure Record for a Monitoring Period, in accordance with the instructions for Department Form R-17, or in clear and legible records containing all the information required by Department Form R-17.
4.46.4 The licensee or registrant shall maintain the records of dose to an embryo/fetus with the records of dose to the declared pregnant woman. The declaration of pregnancy, including the estimated date of conception, shall also be kept on file, but may be maintained separately from the dose records.4.46.5 The licensee or registrant shall retain each required form or record until the Department terminates each pertinent license or registration requiring the record.4.46.6 Upon termination of the license or registration, the licensee or registrant shall permanently store records on Department Form R-16 or equivalent, or shall make provision with the Department for transfer to the Department.Colorado Register, Vol 37, No. 14.
