Section 463.242 - Preparation by Board of basic form of application and supporting documents for licensing by county and city1. The Board shall investigate the information required by each county and city which licenses gaming, and shall prepare a basic form of application and supporting documents which must include:(a) A statement concerning the applicant's personal history;(b) A questionnaire concerning the amount and sources of the capital to be invested in the establishment, and the applicant's personal financial statement; and(c) Any other information which the Board finds is required by all or a majority of these counties and cities or the State. In preparing these basic forms the Board shall consider the form which it requires of applicants for a state gaming license, and to the greatest extent possible shall so design the forms for local use that a copy or partial copy of the State's form supplies the information required.
2. Each county and city which licenses gaming shall use the basic documents whose form is prescribed by the Board, and may require only such additional information as is not contained in those documents.
