Section 50-32-403 - Common reporting form(1) The department of justice must provide a common reporting form for the report required under 50-32-401.(2) The form must contain the following information:(a) name of the substance;(b) quantity of the substance sold, transferred, or furnished;(c) the date the substance was sold, transferred, or furnished;(d) the name and address of the person buying or receiving the substance; and(e) the name and address of the manufacturer, wholesaler, or retailer.En.
