The reports shall be submitted to the Center for the Coordination of Services to Persons Suffering from Alzheimer’s Disease attached to the Department of Health, [on] especially designed forms and [shall] include all relevant information to study and follow-up these cases. In processing the data, the Center for the Coordination of Services to Persons Suffering from Alzheimer’s Disease attached to the Department of Health, shall substitute the name of the patient for an alphanumeric code. The reports shall subsequently be classified as “confidential” and be filed in a restricted access area.
