The Board may revoke a license, after having given written notice of the violations incurred and hearing, when:
(a) The operator is mentally or physically unfit and his/her incapability is proven before the Board by a medical expert.
(b) The operator has incurred violations of this chapter.
(c) The operator has incurred gross negligence in the practice of his/her occupation in the judgment of the Board.
(d) The license had been obtained by fraudulent means or was granted by mistake.
(e) The operator is a narcotic drug addict or a habitual drunk according to the standards provided for those purposes under the collective bargaining agreement between the agency and the union to shich he/she belongs.
