The Criminal Record Certificate must contain the following information:
(1) Full name of the person for whom the certificate is issued.
(2) Case number and court where sentence was pronounced.
(3) Date of sentence.
(4) Offense for which the person was convicted as well as the jurisdiction in which the guilty verdict is filed.
(5) Penalty imposed.
(6) If sentence is in the stage of appeal.
(7) Date of certificate.
(8) Signature of the official issuing the certificate.
The Criminal Record Certificates shall also include a notice that the same shall not include convictions for misdemeanors, if more than six (6) months have elapsed since the sentence was served or convictions for felonies if more than five (5) years have elapsed since the sentence was served.
