Rule 6-415 - Petition and Order for Funeral Expenses

When a petition for funeral expenses is required by law, it shall be filed in the following form:

I hereby request allowance of funeral expenses and I state that:

(1) The expenses are as follows (or as set forth in the attached statement or invoice): __________.

The May 9, 2000, order, in the form, deleted the prefix "19 in the year.

The November 8, 2005, order amended the cross reference.
