Form U.W. 6-69, "Application to Deepen an Existing Well", should be submitted prior to deepening a recorded well when the yield will not be increased above the original appropriation and when the use or uses will not be changed. The form is self-explanatory, and any unknown information can be obtained from the original Permit and Statement of Completion. The present depth and proposed depth must be shown, and the reason for deepening the well explained. The form must be signed and dated by the applicant.
