Section 31-5-4 - [Effective until 7/1/2024] Criteria for Provider Approval4.1. Except as provided for in section 5 of this rule, in order to be an approved provider, a person or organization shall submit to the Board an application for approval on a form provided by the Board. All provider applications and documentation submitted to the Board shall be typewritten and in English.4.2. The approval of the provider expires one (1) year after it is issued by the Board and may be renewed upon the filing of the required application.
