Section 388-835-0075 - What if an individual is transferred between facilities?(1) When an individual is transferred between facilities, all essential information concerning the individual, their condition, regimen of care and training must be transmitted, in writing, by the sending facility to the receiving facility at the time of the transfer.(2) "Transferred between facilities" means transferred from: (b) An ICF/ID to a hospital;(c) A hospital to an ICF/ID; or(d) An ICF/ID or hospital to alternative community placement.Wash.
