Section R510-401-8 - Waiver Requests for Respite and Supplemental Services(1) Any waiver of a limit on respite or supplemental services shall be at the discretion of the local area agency.(2) A request for a waiver of a limit on respite or supplemental services shall be submitted in writing to the local area agency and reviewed by the local area agency's director or the local area agency director's designee. A waiver request shall demonstrate the local area agency's efforts to access other sources of services or available funds.(3) A waiver request, and accompanying documentation, shall be maintained in the individual's client file.(4) Any approved waiver request shall be submitted in writing to the division.Utah Admin.
