Section 74:21:02:68 - Contract provisions

A contract for system operation pursuant to § 74:21:02:67 must contain the following provisions which shall be considered by the board in granting or denying approval:

(1) The parties involved, including names, addresses, and phone numbers of each;
(2) The specific starting and expiration dates of the contract;
(3) The minimum number of visits each month and the minimum time of each visit to be spent on-site by the contract operator;
(4) The duties and responsibilities of each party involved; and
(5) The signature of each party to the contract.
