Section 67:54:07:11 - Required cost reports

Each year the TBI service provider must complete and submit a cost report to the Department of Human Services by November 15. The cost report must be on the cost reporting form available from the Department of Human Services. The reporting period must cover the 12-month period of July 1 through June 30.
