Any person, including an owner, performing plumbing work, except for plumbing repair work, shall report the work to the commission upon a plumbing installation certificate form. The form shall be obtained from the commission at a cost of $5 and it becomes the responsibility of the certificate holder. Upon commencement of any plumbing installation which is subject to inspection, the person responsible for the installation shall send the white copy to the office of the commission with the appropriate fee attached. The yellow copy shall be retained by the person responsible for the installation. The pink copy shall be provided to the owner of the premise. The green copy shall be posted at the job location.
