Section 67-412 - Employer's Report of Injury, Form 12MA. The employer's representative shall report to the Commission injuries reported by the employer pursuant to R.67-411C(1).B. This report shall be made in accordance with R.67-416 within ten days of closing by the employer's representative.C.
