Section 670:15-35-4 - Employer application

(a) Information regarding subscribing to the Driver Status Notification System may be obtained by calling 405-425-2300, or online at service.ok.gov. Inquiries may also be by made by e-mail at dsns@service.ok.gov.

(b) An employer desiring to subscribe to the Driver Status Notification System shall submit to Service Oklahoma an application provided by Service Oklahoma along with the following documentation:

(1) For each motor carrier, an MCS150 (Motor Carrier Identification Report);

(2) For public employers, (school district, state agency, or political subdivision) a letter from the administrator on the letterhead from public entity;

(3) Any other employer of motor vehicle drivers, a tax identification number or other documentation which provides proof of the legitimacy of the established business enterprise; or

(4) For any employer, other documentation as requested by Service Oklahoma which provides proof of the legitimacy of the established business enterprise.

(c) The applicant employer shall agree to maintain the security of any information received from Service Oklahoma in accordance with the federal Driver's Privacy Protection Act (DPPA), 18 U.S.C., Sections 2721 through 2725.

(d) No employer shall participate in the Driver Status Notification System unless and until approved by Service Oklahoma to receive information from the System.

(e) The annual subscription fee for each employer participating in the Driver Status Notification System shall be Fifty Dollars ($50.00). State Agencies shall not be charged a fee to enroll and participate in this program.
