Section 75-03-36-09 - [Effective until 4/1/2024] Personnel files

The child-placing agency shall have a personnel file for each employee that includes the following:

1. The application for employment, including history of employment.
2. Reference letters, telephone notes, or both from former employers and personal references.
3. Applicable professional credentials and certifications.
4. Annual performance evaluations.
5. Orientation and training records.
6. Record of salary and salary adjustments.
7. Employee's first and last dates of employment.
8. Reason employment ended.
9. Copies of criminal history record investigation results and child abuse and neglect index findings.
