Section 6173.4 - Reports

(a) A provider agency receiving funds pursuant to this Part shall provide to the division a quarterly report which shall include, at a minimum:

(1) the number of livery vehicle drivers served;

(2) the nature of the program services provided; and

(3) the costs of the program services provided.

(b) A provider agency receiving funds pursuant to this Part shall provide to the division a final written report within 30 days after the conclusion of program services which shall include, at a minimum:

(1) a description of how the services provided enhanced livery vehicle safety; and

(2) the nature and costs of program services provided.
