N.J. Admin. Code § 8:34-4.5

Section 8:34-4.5 - Written plan and reports

(a) An applicant shall submit a completed application for approval of administrative intern program request in the form at Appendix B, incorporated herein by reference, to the Board for approval prior to beginning the program.

(b) The preceptor shall maintain progress reports for an administrative intern in the form at Appendix C, incorporated herein by reference, for each quarter of the required hours of the training program and shall sign and submit the completed progress reports to the Board.

(c) The progress reports described in (b) above shall include the following:

1. Subjects covered and hours spent in each department;

2. Comments on the monthly internship logs as to accuracy and completeness;

3. Progress of the administrative intern; and

4. Identification of problems, if any.

(d) The administrative intern shall maintain monthly logs of work activities and shall quarterly submit these to the Board.

(e) The administrative intern shall co-sign and submit the progress reports described in (b) above to the Board.

(f) A preceptor for an administrative intern shall submit the progress reports described in (b) above to the Board within 15 working days of the end of each quarter.

(g) The preceptor for the administrative intern shall complete and sign a certification of program completion in the form at Appendix D, incorporated herein by reference, and shall submit it to the Board within 15 working days of the administrative intern's completion of the program.

(h) If a preceptor fails to submit the report(s) required in a timely manner as indicated in (f) and (g) above, the Board may require the administrative intern to forfeit all credit for the training accumulated on the report(s) for that period.
