Applicants shall submit the proposed VCM Plan, an application for a highway occupancy permit, and the appropriate application fee to the Operations Permit Office, pursuant to the requirements of N.J.A.C. 16:41. Applications shall be submitted to the address found at N.J.A.C. 16:41F-3.1. Permit application forms (MT-120A) can be found at: http://www.state.nj.us/transportation/eng/forms/index.shtml#maintenance.
