Section Saf-C 3104.05 - Application for Driver Education Certificate(a) Each applicant for a driver education certificate shall complete and submit the form DSMV 635, "Application for Driver Education Instructor Certificate" (rev 02/18).(b) The applicant shall submit an official copy of his or her college transcripts.(c) The applicant shall provide a certified copy of his or her motor vehicle record from any state in which the applicant has held a driver license and from any other state in which the applicant has obtained a motor vehicle conviction.(d) Pursuant to RSA 263:44-b, the applicant shall provide a certified copy of his or her criminal record from this state and from any other state in which the applicant has obtained a criminal conviction.(e) The applicant shall sign and date the form under the penalty of unsworn falsification.(f) If requesting a driver education provisional certificate, a fee of $25 shall be submitted along with the application.(g) If requesting a driver education standard certificate, a fee of $40 shall be submitted along with the application.N.H.
