Section Saf-C 3104.04 - Application Forms(a) Application forms may be obtained from: Department of Safety

(b) A completed application shall include the following: (1) Proof of driving experience as set forth in Saf-C 3103.02;(2) Report of physical examination as set forth in Saf-C 3103.03;(3) Successful completion of written and on-road examinations as set forth in Saf-C 3103.04;(4) Criminal history record check;(5) Fingerprints, upon initial application only, pursuant to RSA 263:44-b; and(6) Motor vehicle record from any state the applicant has held a driver license.(c) Completed application forms shall be submitted to the: Department of Safety
