Section Saf-C 311.14 - Restoration Notice

(a) The department's restoration notice shall contain the following:

(1) Company or person's name, address and federal employer identification number, if applicable;

(2) Date of restoration;

(3) Account number;

(4) Date of suspension notice; and

(5) Indication of action taken and license/permits restored.

(b) The commissioner or his/her designee shall sign the form.
