Section Saf-C 311.13 - OHRV Refund Assignment(a) An applicant shall submit the following on the OHRV refund assignment: (1) Applicant's name and federal employer identification number, if applicable;(2) OHRV club's legal name and address;(3) Statement of assignment of rights to the club;(4) Number of gallons requested for refund; and(5) The year, make, model and registration number of the off highway road vehicle.(b) The preparer shall sign and date the form.N.H.
