On a form provided by the commission, an applicant for an occupational license as a greyhound racing announcer, chart writer, lure operator, clerk of scales or paddock judge shall, in addition to the application form specified in Pari 806.05, supply the following information:
(a) The name of the applicant as it appears on the form specified in Pari 806.05;(b) The name, address and telephone number of the person, corporation or association licensed under RSA 284: 15-a recommending the applicant for occupational licensure;(c) Whether or not the applicant has successfully completed on-the-job-training during 150 races under the supervision of a current or former occupational licensee in the occupational category in which the applicant seeks;(d) If the answer to the foregoing question is yes, the name of the supervising occupational licensee or former occupational licensee and the date and location of the supervised training;(e) Whether or not the applicant has vision which is at least 20/40 in both eyes, or corrected to 20/40 in both eyes;(f) If the answer to (e) above is yes, the applicant's uncorrected vision;(g) The applicant's signature on the statement set forth in Pari 806.28 below; and(h) The date of the signature described in (g) above.N.H.
