Section Pari 806.15 - Supplemental Application Form for Greyhound Assistant Race StarterOn a form provided by the commission, an applicant for an occupational license as an assistant race starter shall, in addition to the application form specified in Pari 806.05, supply the following information:
(a) The name of the applicant as it appears on the form specified in Pari 806.05;(b) The name, address and telephone number of the person, corporation or association licensed under RSA 284: 15-a recommending the applicant for occupational licensure;(c) The name, address and telephone number of the occupationally licensed race starter who has agreed to supervise the applicant;(d) Whether or not the applicant is able to lift at least 60 pounds in weight and carry it a distance of 20 feet;(e) Whether or not the applicant is able to run 100 yards in 20 seconds or less;(f) The applicant's signature on the statement set forth in Pari 806.28 below; and(g) The date of the signature described in (h) above.N.H.
