Section Hum 202.03 - Complaints of Discrimination Other than Employment Discrimination(a) A complaint of discrimination not including employment discrimination shall be made on a form provided by the commission or in a letter containing the following: (1) The complainant's: c.Home telephone number; and(2) If the complaint is filed by the attorney general, the name of the attorney general;(3) The name, full mailing address and telephone number of each respondent;(4) Whether the alleged unlawful discriminatory practice was in housing, public accommodations or commercial property;(5) Each of the following bases on which the practice complained of are alleged to have discriminated: k.Sexual orientation; andl.Retaliation for making a complaint of unlawful discrimination;(6) The date of the alleged most recent act of discrimination or of continuing discrimination;(7) A statement of the alleged discriminatory practice complained of;(8) A statement of the facts alleged to constitute the discriminatory practice;(9) Whether the complainant has filed any other action based on the same allegation;(11) The verified signature of the complainant.(b) If the commission receives a complaint which appears to allege discrimination but does not include all of the information called for by (a) above, commission staff will contact the complainant and assist the complainant in providing the required information.N.H.
