Section Fis 812.03 - Record Keeping and Reporting Requirements(a) Permittees shall maintain a current record of all wildlife received and/or released which shall include the name and address of the permittee.(b) For each animal received the record shall include the following: (2) Date wildlife was found;(3) Location where wildlife was found;(5) Name, address and telephone number of individual from whom acquired;(6) Reason for admission;(7) Date(s) of veterinary evaluation and/or treatment;(9) Date of disposition; and(c) Permittees shall maintain records for 5 years.(d) Permittees shall submit to the department a copy of all records kept pursuant to Fis 812.03(b) each year by January 31 of the following calendar year.(d) Wildlife rehabilitation facilities and records shall be available for inspection by fish and game department personnel at any reasonable hour for the purpose of ascertaining compliance with these rules.(e) Department personnel shall remove wildlife from the custody of the permittee if it is discovered that the permittee is not in compliance with Fis 812.02(a), (b), (e), (f), (g), or (h) or that is determined to pose significant risk as described in Fis 812.01(h).N.H.
