Section Env-Wq 1205.15 - Reports of Noncompliance(a) The user shall provide notice to the WRBP in accordance with this section in the event of any of the following:(1) A discharge violation detected through analysis;(2) Unanticipated bypass or slug discharge; or(3) Upset that exceeds applicable pretreatment standards.(b) When required to notify the WRBP, the user shall provide the following:(1) Oral notice by telephone within 24 hours of the time the user becomes aware of the event; and(2) A written report within 5 business days of the time the user becomes aware of the noncompliance.(c) The written report required by (b)(2), above, shall include: (1) A description of the event and its cause;(2) The duration of the event, including exact dates and times;(3) If the event has not been corrected, the anticipated time it is expected to continue; and(4) Steps taken or planned to reduce, eliminate, and prevent recurrence of the event.N.H.
