Section Env-Hw 609.03 - Registration Modification

A transporter shall notify the department by providing updated information in writing within 15 days of any of the following:

(a) A name change for the transporter;

(b) A change in physical location or mailing address of the transporter;

(c) A change of company contact of the transporter;

(d) A change in telephone number of the transporter; and

(e) A change in the EPA identification number of the transporter.
