Section Env-Dw 305.06 - Preliminary Report: Cover Page The cover page required by Env-Dw 305.05(a)(1) shall contain the following:
(a) Information to identify the persons involved, specifically the name, mailing address, email address, and daytime telephone number of each of the following: (1) The water system owner, or, if there is no owner at the time the preliminary report is submitted, the project owner and a description of the plan to transition ownership of the CWS in the future;(2) The owner of the well site property;(3) The individual responsible for responding to questions from the department regarding the preliminary report; and(4) The person responsible for performing the pumping test and water quality sampling program;(b) A description of the purpose of the new well and who is or will be served by the CWS; and(c) The source capacity requirements for the small CWS established in accordance with Env-Dw 405, including: (1) Irrigation water use requirements; and(2) Estimated water use based on the water use categories specified by Env-Dw 405.N.H. Code Admin. R. Env-Dw 305.06
