"Significant deficiency" means a deficiency that can directly and adversely affect a public water system's water quality or that can reduce the water system's reliability and ability to deliver safe drinking water to its customers, including but not limited to defects in design, operation, or maintenance, or a failure or malfunction of the source(s), treatment, storage, or distribution system(s) that could cause or allow the introduction of fecal or microbial contamination into the water delivered to consumers.
