Section Env-A 1804.06 - Information Required(a) In any notification required by this part, the O/O shall: (1) Provide the information required by 40 CFR 61.145(b)(4); and(2) Present that information in the format specified in 40 CFR 61.145(b)(5), available as the "Asbestos Demolition/Renovation Notification Form, 2016-05-31".(b) The O/O shall provide any additional information that is not specifically required by this part but that is necessary to completely describe the scope of the project to the department separately from the required information by first-class USPS mail, commercial delivery service, or hand delivery as specified in Env-A 1801.04(a).(c) Hours of operation on site shall be included on the notification document for purposes of inspection.N.H.
