Section 474-1-004 - APPLICATIONS

Any Nebraska resident may contact the Department by telephone, in writing or in person to obtain information, explore eligibility or to apply for services on the person's own behalf or as an authorized representative of another person. Applications may be submitted in person, by mail, by fax or by electronic submission. An applicant must submit a completed, signed and dated application on an approved Department form. An applicant may request assistance from the Department when filling out an application.

004.01INTERVIEWS. An applicant must participate in an interview as part of the initial eligibility determination. Interviews may be conducted face-to-face or telephonically. The Department determines if a face-to-face interview is necessary. If an applicant is unable to participate in a face-to-face interview in a local office, the Department and the applicant will discuss a mutually acceptable time and place for an interview such as a hospital, a senior or community center or the applicant's home. At least one of the following must attend the interview:
(A) An adult applicant;
(B) The applicant's legal guardian or conservator; or
(C) An adult representing the applicant.
