Section 180-3-021 - AMENDMENT OF LICENSES AT REQUEST OF LICENSEE

Applications for amendment of a license must be filed according to 180 NAC 3-010 and must specify the respects in which the licensee desires their license to be amended and the grounds for such amendment.
