Section 26.04.01.26 - Monthly Operating Reports

A. The Approving Authority may require suppliers of water to submit a monthly operating report not later than 10 days after the end of the reporting period.

B. The Approving Authority shall specify the information required and the format of the monthly operating reports for each water supply system.

C. The current monthly operating report shall be kept at the water treatment plant, and all entries shall be made on a daily basis.

D. Reportable incidents shall be noted in the monthly operating report of addendums attached to it.

E. The superintendent in charge of daily operations shall complete the monthly operating report, certify that the report is truthful to the best of his knowledge and belief, and sign the operating report at the end of the day, shift, work period, or at an interval specified by the Approving Authority. The supplier of water shall be responsible for submitting the report within the required time limit.

F. The Approving Authority shall report to the Board any irregular, untruthful, or misleading information reported on the operating report.
