Section 10.44.24.04 - Malpractice Insurance Requirement for Initial Licensure and RenewalA. An applicant for a retired volunteer dentist's or dental hygienist's license and an applicant for the renewal of a retired volunteer dentist's or dental hygienist's license shall be covered by malpractice insurance.B. If the malpractice insurance consists of a personal policy of malpractice insurance, the dentist or dental hygienist shall provide the Board with evidence of the insurance which shall consist of: (1) The name of the insurance provider;(2) The name, address, and telephone number of the insured's agent, or if the insured has no agent, the address and telephone number of the insurance provider;(4) The amounts of coverage; and(5) The expiration date of the policy.C. If the malpractice insurance is provided from or through the institution at which the applicant is to provide volunteer services, the dentist or dental hygienist shall provide the Board with evidence of the insurance which shall consist of: (1) The name of the insurance provider;(2) The amounts of coverage;(3) The expiration date of the policy or coverage; and(4) A letter from the institution verifying the intent of the institution to provide malpractice insurance.Md.
