Section 10.03.01.14 - Death Certificate - AmendmentA. In order for the name of the deceased, the cause of death and medical certification, the manner of death, the date and time of death, and the place where the death occurred to be amended on a death certificate, the facts necessitating the amendment shall be certified to the Secretary by the physician, physician assistant, or nurse practitioner who filled out and signed the death certificate, or by the medical examiner.B. When a death certificate is filled out and signed by the medical examiner, only the medical examiner may certify to the Secretary the facts necessitating an amendment on the death certificate to the name of the deceased, the cause of death and medical certification, the manner of death, the date and time of death, and the place where the death occurred.Md.
