The complaint must contain, insofar as known, all of the information specified in a form provided by the Board. The complaint must also include a Concise Statement of Facts, which is a clear and concise statement of the facts constituting the complaint, set out in separate numbered paragraphs. The complaint may also include, but is not required to include, attached documentary evidence in support of the factual allegations made in the complaint.
